What to expect

Prior to first visit

• Book your initial appointment

• Set up client account

• Fill out intake form

• Know your health insurance coverage information if available.

Your First Visit

• Wear comfortable, loose fitting clothes where possible

• Arrive 15 minutes early if possible, expect to wait for Dr. Jillian to come get you

• Review Intake form, physical exam, report of findings, treatment options, plan your treatment type, review schedule of treatment and associated fees

• Treatment if determined appropriate

Follow Up Visits

• Every initial treatment has minimal one follow up visit.

• Recommended treatment plan discussed at initial visit should be followed for best outcomes.

• After 12th visit, there is a reassessment visit with associated fees.

You are subject to a new "initial visit" if;

• New physical complaint or new traumatic event occurs

• No treatments in 6+ months.
